Filing a College Transcript

If you are a senior at Simeon and you need an official transcript sent off to a college then you need to see Ms. Gibbs in the Senior Suite (Rm. 149).
1. Go to Room 149
2. See Ms. Gibbs to get your TRANSCRIPT REQUEST FORM
3. Fill out the TRANSCRIPT REQUEST FORM with the mailing address of the colleges you want to receive your transcripts
4. Hand the form back to Ms. Gibbs
5. Make sure you get your college packets ready
a. Address the brown envelope to the college
b. Put all relevant materials in college envelope and mail off
*You are responsible for mailing off the other materials to the colleges
* There are over 400 seniors at Simeon, so any way that you can assist Ms. Gibbs would be greatly appreciated. Thank you
Alumni Transcript Request

If you are a Simeon Alumni and need a transcript for the following reason:
• Students who have graduated within the last 8 years (2000 - Present)
• For Colleges/GED Programs
• Employment
• Government Requests
you may contact Ms. Gray to receive a copy of your official transcript.
Steps to obtain your official transcript:
1. Go to Room 238
2. See Ms. Gray to get your TRANSCRIPT REQUEST FORM
3. Fill out the TRANSCRIPT REQUEST FORM with the mailing address of the institution transcript is being sent to.
4. Hand form back to Ms. Gray
5. The cost for each transcript is $2.00

Unofficial Transcript Requests
Any current student that would like an unofficial transcript may see the Counseling Department at Simeon to obtain their current transcript. The unofficial transcript may only be given to current students or parents of current students.
Frequently Asked Questions
What if my classes are not showing up on my transcript or my grades were entered wrong?
Follow these steps to try and resolve the situation:
1. Contact the teacher of the class in question (E-mail or speak directly)
2. If you cannot contact the teacher of the class, contact the department chair
3. If the situation is not resolved then speak with Assistant Principal Lusbourgh (Academic Dean) about your situation
4. If the situation is still not resolved, then speak with the Principal.
Once the problem is solved you can finish the process by following these steps:
5. Get a GRADE CHANGE FORM from the teacher to fill out together
6. Make 3 copies of the GRADE CHANGE FORM
7. Give one copy to Mr. Stapleton, the grade coordinator
8. Keep one copy
9. Give one copy to your counselor, for record-keeping
