Instructions for Completion:
Please complete this application with an adult!
  • Answer all required information honestly and truthfully (all information will be verified).
Submit your on-line application by Thursday - JANUARY 28, 2016 (11:59pm) AND print the Confirmation of Submission page .
Submit your printed Confirmation of Submission page to your Counselor to complete your application.
  •   If we do not receive an on-line application by Thursday - JANUARY 28, 2016  (11:59pm), and the hard copy information listed below            
                      from your Counselor by Wednesday - February 17, 2016 your application WILL NOT BE PROCESSED.

        Information provided on-line will be used to begin the process.     
Hard copies of documentation are needed to finalize the process.

Applicants will receive a letter indicating their current status of admissions.
(Failure to submit current/correct mailing or email addresses will jeopardize your admission.)

***NWEA MAP Test 

All students who are applying to grade 9 for the 2016-2017 school year should have taken the Northwest Evaluation Association Measures of Academic Progress (NWEA). The NWEA MAP is used to determine eligibility for students to apply to Simeon. Please provide us with your 7th grade NWEA Scores (or Stanines and Percentiles from any other Standardized tests you took in 7th grade) so that we may take a preliminary look at your academic records.

If the student has already taken a nationally normed measure that we accept (Stanford Achievement Test 10, Iowa Tests of Basic Skills (ITBS) Kaufman Test of Educational Achievement 2, Woodcock Johnson Achievement 3, and Wechsler Individual Test 3, we will accept these scores in lieu of the NWEA.
